Hi Dan - Grease will often allow you to tighten your bolts up more, so if you're currently not using grease on your brake bolts it could be that they feel tight just because the bolt is binding in the frame rather than it actually being because everything has tightened down. If the cylinder washers on your brake are old, then they can deform and mean that you can't clamp the cylinders properly. Replacing the washers may help. You could also check that your bolts aren't bottoming out in your frame before they're fully tight. Without having more information about the frame/fork/brake/clamps it's difficult to offer more help, sorry!
